  1. NPDB (National Practitioner’s Data Bank) Report — Current or past malpractice claims, license actions, or various other reports
  2. 5-Year Work History and Verification of Dental Education Graduation
  3. Verification of a Current State License in Good Standing
  4. DEA/CDS License Verification — Allows dentist to write prescriptions
  5. Current Malpractice Insurance Verification
  6. Government Reporting Inquiry — Medicare fraud, Medicare opt-out, etc.
  7. Regular review to verify all criteria remain in good standing
